Dr. Kaplan Video with Optin Form at End of Video

The optin form comes up at the end of the video (but with no call to action)

I would suggest placing text at the top of the form - like, "Send us your info so we can schedule your consultation" or something similar. Give them a REASON to want to give you their info.
